Radiography has long been an integral component of endodontics, serving as an indispensable diagnostic, therapeutic, and follow-up tool.Radiology remains the cornerstone of endodontic diagnosis and treatment. From the early use of conventional periapical films to the current era of digital and volumetric imaging, radiographic innovation continues to enhance the precision and predictability of endodontic therapy. While 2D imaging remains adequate for most routine cases, advanced 3D modalities-particularly CBCT-have transformed the diagnostic landscape, allowing clinicians to visualize intricate canal systems and periapical anatomy with unprecedented detail. Ongoing research into MRI, ultrasonography, and functional imaging heralds the next frontier in endodontic diagnostics, potentially offering non-ionizing alternatives with superior soft-tissue resolution. Ultimately, judicious selection of imaging modalities, guided by the principles of ALARA and evidence-based practice, ensures optimal patient outcomes and paves the way for continued advancement in endodontic radiology.
